Comments:
A monitoring inspection was initiated on April 26, 2022 and concluded on April 29, 2022. There were 48 children present with 11 staff supervising and 2 administrative staff present during in person observation. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 3 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on review of staff record and interview, the center did not ensure to obtain a copy of results of a criminal history record information check, a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided.
Evidence: In Staff#3's record (date of hire: 4/11/22) there were no results of a criminal history or sex offender registry check found during the inspection. Interview with staff confirmed results of the out of state searches were not available for review.
Plan of Correction:
The center re-mailed central registry search request as of 4/29/2022. The sexual registry search has been completed and submitted for staff already.

Description:
Based on review of staff records and interview, the center did not ensure to obtain TB documentation within the last 30 calendar days of the date of employment.
Evidence: In Staff#2's record (date of hire: 02/07/22), the TB documentation was dated 12/10/21. In Staff#3's record (date of hire: 4/11/22), the TB documentation was dated 11/15/21. Interview with staff confirmed these TB documentation were the most recent for review during this inspection.

The center ensure both staff completed TB test and files are updated.

Description:
Based on review of staff records and interview, the center did not ensure to obtain documentation to demonstrate certification, and experience required by the job position.
Evidence: During the inspection on 4/26/22, Staff#1 was observed in the classroom alone with children in care and identified as a lead teacher position but there was no available documentation to verify lead qualification in the record. Interview with staff confirmed Staff#1 documentation to demonstration qualification was not available for review during the inspection.

The center replaced with a lead teacher until the assistant staff is CDA certified.

Description:
Based on observation, the center did not ensure all hazardous substances were kept in a locked place
Evidence: 1. During the inspection on 4/26/2022, in the center's Infant C classroom a cabinet containing multiple cleaners with the labels "warning" and "keep out of reach of children" was observed unlocked.
2. There was observation in the preschool classroom in which a floor cleaner was observed in the bathroom area with the label "keep out of the reach of children".
Plan of Correction:
The center purchased locks to in sure this does not happen again.
